Diabetic eye - what are the symptoms and how does it affect us?

1. How does diabetes affect our eye health?

Diabetes-induced eye conditions include diabetic retinopathy, diabetic macular edema, cataracts and glaucoma.

Depending on the duration of diabetes and its control, diabetes can cause damage to the eyes that can lead to poor vision or even blindness. Only rigorous diabetes control can prevent diabetic eye disease and its worsening.

The best ways to manage your diabetes and keep your eyes healthy are

  • manage your blood sugar, blood pressure and cholesterol correctly
  • quit smoking
  • have a complete eye examination every 6 months

Often, incipient biabet lesions in the eye do not produce warning signs of diabetic eye disease or vision loss. A thorough eye exam, helps your doctor find and treat eye problems early, often before major vision loss occurs.

2. How does diabetes affect my eyes?

Diabetes affects your eyes when your blood glucose, also called blood sugar, is too high.

In the early stages of the disease, you are unlikely to lose vision due to high blood sugar. People sometimes have blurred vision for a few days or weeks when they change their diabetes care plan or medications. High glucose can change fluid levels or cause swelling of eye tissues. This type of blurred vision is temporary and goes away when glucose levels get closer to normal.

If blood glucose remains high over time, it can damage the small blood vessels at the back of the eye. This damage can start before the diabetes dagnostic is established, when blood glucose is higher than normal but not high enough to be diagnosed with diabetes. Damaged blood vessels can leak fluid and cause edema. New, weak blood vessels may also start to grow. These blood vessels can bleed into the middle part of the eye, lead to scarring or cause dangerously high intraocular pressure.

The most serious diabetic eye diseases start with blood vessel problems. The four eye diseases that can threaten your vision are diabetic retinopathy, diabetic macular edema, glaucoma, and cataracts.

1. Diabetic retinopathy

The retina, the inner layer of the eye, or the vision receptor, senses light and converts it into signals that the brain decodes so that humans are able to see the world around them. Damaged blood vessels can damage the retina, causing a disease called diabetic retinopathy.

In early diabetic retinopathy, blood vessels may weaken, pump or leak fluid that accumulates in the retina. This stage is called non-proliferative diabetic retinopathy.

If the disease worsens, some blood vessels close, causing new blood vessels to grow or proliferate on the surface of the retina. This stage is called proliferative diabetic retinopathy. These abnormal new blood vessels, called neovessels, can lead to serious vision problems.

2. Diabetic macular edema

The part of the retina you need to read, drive and see faces is called the macula. Diabetes can lead to swelling of the macula, which is called diabetic macular oedema. Over time, this disease can destroy the vision in this part of the eye, leading to partial vision loss or blindness. Macular oedema usually develops in people who already have other signs of diabetic retinopathy.

3. Glaucoma

Glaucoma is a group of eye diseases that can affect the optic nerve - the bundle of nerves that connects the eye to the brain. Diabetes doubles the chances of glaucoma, which can lead to vision loss and blindness if not treated early.

4. Cataract

The lenses inside our eyes (crystalline lenses) are clear structures that help us see clearly, but tend to become opaque as we age. People with diabetes are more likely to develop clouding of the lens, called cataracts. People with diabetes may develop cataracts at a younger age than people without diabetes. Researchers believe that high glucose levels cause deposits to build up in the lens.

Diabetic retinopathy - Approximately one in three people with diabetes who are over 40 already have some signs of diabetic retinopathy. Diabetic retinopathy is the most common cause of sight loss in people with diabetes. However, each person's outlook for the future depends largely on proper care. Finding and treating diabetic retinopathy early can reduce the risk of 95% blindness.

Glaucoma and cataracts - The chances of developing glaucoma or cataracts are about twice those of a person without diabetes.

3. Care sunt simptomele bolii oculare diabetice?

Often there are no early symptoms of diabetic eye disease. You may have no pain and no change in vision because the changes start to develop inside the eyes, especially with diabetic retinopathy.

When symptoms occur, they may include

  • blurred vision
  • poor colour vision
  • frequent change in vision - sometimes from day to day
  • dark areas or loss of vision
  • pete sau miodezopsii
  • bright flashes

We recommend that you see a specialist if you have any of these symptoms.

4. Cum tratează oftalmologii bolile oculare diabetice?

Your doctor may recommend eye exams more often than once a year, along with managing your diabetes. This means managing your diabetes, which includes glycosylated hemoglobin, blood pressure and cholesterol; and quitting smoking.

Your ophthalmologist may treat your eyes with anti-VEGF drugs such as aflibercept, bevacizumab or ranibizumab. These drugs block the growth of abnormal blood vessels in the eye. Anti-VEGF drugs can also stop fluid leakage, which can help treat diabetic macular oedema. These anti-VEGF drugs are injected into the eye. You'll have more treatments in the first few months, then fewer treatments after you finish the first round of therapy. The procedure is performed with local anaesthetic so you don't feel pain. The needle is about the thickness of a human hair.Anti-VEGF treatments can stop further vision loss and improve vision.

Laser treatment

Vitrectomy

Vitrectomy is surgery to remove the clear gel that fills the eye, called vitreous gel. The procedure treats vitreous hemorrhage or tractional retinal detachment induced by proliferative diabetic retinopathy. A retina that completely loosens or detaches can cause blindness.
