Ocular Ultrasound: how it works
and when is it recommended?

Ocular ultrasonography, ocular ultrasound or "B-scan" is a rapid, non-invasive diagnostic method used in routine practice to assess the anatomical integrity of the eye and possible pathologies. It provides additional information especially when direct visualisation of ocular tissues is not possible due to lack of transparency in corneal, crystalline or vitreous opacities.

1. How does ultrasound work?

By definition, ultrasound has frequencies above 20 kHz, above the human audible limit. Ocular ultrasound uses waves with frequencies of about 10 MHz produced by a small probe that is placed on the closed or open eye, as appropriate. With topical anaesthesia in the case of the open eye, or without anaesthesia with the eye closed, a gel is applied between the probe and the eye to remove air bubbles. This probe collects the echoes ("echo") of tissues that it translates into a two-dimensional image of the eyeball.

The denser the tissue, the brighter it will appear (hyperechogenic) and conversely, the more aerated, the less bright (hypoechoic). If the tissue is dense enough, it will also have a cone of shadow behind it, which is common in some tumours or intraocular foreign bodies. This non-invasive and painless technique can easily be performed in the office, during consultation, at the patient's bedside or in the operating room. Moreover, ultrasound can be easily performed on children without sedation.

2. When is an eye ultrasound recommended?

  • Annually, during the ophthalmological consultation, as a screening method
  • Atunci când percepem „muște volante” sau tulburări în câmpul vizual
  • At regular intervals, once every 3 months, for diabetic patients with severe eye damage.
  • In the event of sudden loss of vision
  • In case of severe headaches
  • In case of eye or head injury

3. What can we discover from an eye ultrasound?

  • Posterior vitreous detachment: often accompanied by "flashes" or "flying flies" in the visual field, this is not an emergency in itself, but can be complicated in some cases with vitreous hemorrhage and rhegmatogenous retinal detachment.
  • Vitreous disorders: in the case of dehydration and collagen degeneration in the vitreous body, these are perceived when looking at a light-coloured background as "floating spots" or "clouds". They may or may not be accompanied by posterior vitreous detachment.
  • Papillary oedema: having several causes, from intracranial hypertension, papillary drusen or various infections and inflammations, it appears as an elevated optic disc on the retina, with or without adjacent hyperechogenic calcification.
  • Intraocular foreign body: in the case of trauma, ocular ultrasound is mandatory if it can be performed safely. Foreign bodies made of metal, glass or wood with a posterior shadow cone can be detected.
  • Dislocated lens or lens implant: also following trauma, the intraocular lens can be seen in the vitreous cavity, out of its normal position behind the iris.
  • Intraocular tumours: both for diagnosis when using the "A-scan" mode and for following the evolution of a tumour by measuring its size, ocular ultrasonography remains an investigation of choice for intraocular tumours such as haemangioma, retinoblastoma or malignant melanoma.
