{"id":1309,"date":"2022-01-03T11:04:23","date_gmt":"2022-01-03T11:04:23","guid":{"rendered":"http:\/\/profmunteanu.ro\/?p=1309"},"modified":"2023-09-05T09:11:35","modified_gmt":"2023-09-05T08:11:35","slug":"blefarita-cu-demodex-de-la-diagnostic-la-tratament","status":"publish","type":"post","link":"https:\/\/profmunteanu.ro\/en\/blefarita-cu-demodex-de-la-diagnostic-la-tratament\/","title":{"rendered":"Blepharitis with Demodex - from diagnosis to treatment"},"content":{"rendered":"<p class=\"wp-block-paragraph\">The last few years have shown us, and colleagues in different corners of the world, that Demodex blepharitis is much more common than expected. The latest statistics show that this parasite is the cause of 45% of blepharitis in general, in people over 60 it is present in 84% of eyelid inflammations. Among young people with diabetes, acne rosacea or a compromised immune system, Demodex infection is the main cause of blepharitis.&nbsp;&nbsp; <\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong><em>Who is it about?<\/em><\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Mites in\nDemodex species are found on the skin of the face, especially on the forehead, cheeks, sides\nsides of the nose, but also on the eyelids and eyelashes. In the\nsurprisingly or not, these ectoparasites live almost permanently on the skin.\nhuman skin. In about 70% of cases, people infected with these\nmites are asymptomatic, i.e. show no signs or symptoms.\nThis is also the main reason why many people live a lifetime\nwith these parasites on their skin without knowing it. However, larger numbers of Demodex mites can cause\nundesirable symptoms and skin problems.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Demodex\nfolliculorum<\/strong>&nbsp;has a length of 0.3-0.4 mm and is\nfound in small hair follicles, especially on eyelashes. Both in its\nimmature and adult stages, it feeds on skin cells.&nbsp;<strong>Demodex\nbrevis<\/strong>&nbsp;is slightly smaller (0.15-0.2 mm) and is found in the glands\nwhich are connected to small hair follicles. Unlike\nDemodex folliculorum, Demodex brevis feeds on sebaceous gland cells.\nBoth mites are senitransparent, almost invisible to the naked eye, but\ncan be seen under a microscope.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Prin \u00eens\u0103\u0219i prezen\u021ba lor, ace\u0219ti parazi\u021bi produc modific\u0103ri tisulare inflamatorii locale , c\u00e2t \u0219i formarea unor dopuri foliculare cu \u00eenfundarea \u0219i atrofierea consecutiv\u0103 a <a href=\"https:\/\/profmunteanu.ro\/en\/meibografia\/\" target=\"_blank\" rel=\"noreferrer noopener\">glandelor meibomiene<\/a> responsabile de stabilitatea filmului lacrimal. <\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong><em>How is the diagnosis made?<\/em><\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The symptoms that\npatients experience are multiple and non-specific: burning eyes, dryness, embarrassment\neye discomfort, congestion. Localised itching sensation along the\nfree edge of the eyelids, at the root of the eyelashes, can be an element of\ndiagnostic guidance. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Prin examinarea la <a href=\"https:\/\/profmunteanu.ro\/en\/biomicroscopia-ultrasonica\/\" target=\"_blank\" rel=\"noreferrer noopener\">biomicroscop<\/a>, se observ\u0103 prezen\u021ba unui man\u0219on de detritus celular (coleret\u0103) \u00een jurul bazei genelor, semn clinic distinctiv \u0219i esen\u021bial pentru stabilirea diagnosticului. La acesta se adaug\u0103 semne nespecifice: edem \u0219i eritem palpebral, madaroza ( pierderea genelor), hiperemie conjunctival\u0103, leziuni corneene. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Reviews\noptional, often unnecessary, examinations may be microscopy\noptical microscopy of a plucked eyelash (direct detection of the parasite) or\nconfocal laser-scanning microscopy (CLSM)<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong><em>Why should we act?<\/em><\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Prin modific\u0103rile produse la nivel local, prin prezen\u021ba \u0219i activitatea sa, acest parazit induce o stare de inflama\u021bie constant\u0103 la nivelul segmentului anterior ocular, manifestat\u0103 prin blefarit\u0103 cronic\u0103, keratite sau conjunctivite repetate. Deseori leziunile provocate la nivel cornean sunt ireversibile \u0219i asociate cu un impact negativ asupra capacit\u0103\u021bii vizuale. Se consider\u0103 c\u0103 acarienii din specia Demodex reprezint\u0103 un ,,trigger\u2019\u2019 pentru diverse bacterii, motiv pentru care frecven\u021ba blefaroconjunctivitelor este mult crescut\u0103 \u00een cazul acestor pacien\u021bi. De asemenea, s-a demonstrat corela\u021bia dintre prezen\u021ba acestui parazit \u0219i modificarea aspectului \u0219i func\u021bionalit\u0103\u021bii glandelor meibomiene, responsabile pentru stabilitatea filmului lacrimal duc\u00e2nd astfel la dezvoltarea chalazionului \u0219i progresia simptomatologiei specifice ochiului uscat.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong><em>How can it be treated?<\/em><\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Firstly, a\nthing to avoid is the application of corticosteroid ointments, which lead to\nincrease the number of parasites. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Time-efficient,\nhas proven to be tea tree oil, which is why at present\nthere are multiple products (wipes, soaps, gels) based on tea tree oil\nused for eyelid hygiene. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In recent years, the treatment with intense pulsed light is considered to have a real effect on the development and multiplication of this parasite. According to the latest studies, this treatment has demonstrated an eradication rate of infection of about 83%, one month after the third treatment session.
